Renewed Individual Care Plan 3.0 in use

Parents and healthcare professionals can download the Individual Care Plan from our website.


After intensive and multi-year research in collaboration with parents and healthcare professionals, the renewed Individual Care Plan 3.0 (IZP) can now be used. A care plan that really matches the wishes and expectations of parents and caregivers.

In an Individual Care Plan, wishes and agreements regarding the care, treatment and quality of life and death of the child are recorded. By using an Individual Care Plan, the care at home, in the hospital and in your own environment (such as at school) can be well coordinated. Care can also be transferred more quickly and easily, because everyone is well informed. This benefits the quality of pediatric palliative care.

Don't think of it as something heavy. You can put everything in it. Not only topics about the end of life, but also how to deal with the person who needs care. That can always be adjusted. This way you can use the plan as a tool to talk to each other about the quality of life of the child.'

Important to know:

  • An Individual Care Plan can be drawn up for all children with a life-shortening or life-threatening diagnosis.
  • The plan is started up and completed by a committed pediatric nurse and the main practitioner or involved pediatrician, together with the child and family.
  • In addition to physical care, the plan pays a lot of attention to the psychological, social, pedagogical and spiritual aspects.
  • Agreements or wishes can always be adjusted or changed.


Getting started with the IZP?

Parents and healthcare professionals can download the Individual Care Plan from our website . To clarify the process of drawing up and working with an ICP, process agreements have also been drawn up and described in the accompanying pocketbook. Within NPPZ II we will start digitizing the IZP to ensure that the plan is always available and up-to-date. Do you have questions about the Individual Care Plan? The renewed IZP was created within the framework of the IZP Project. The research was done by researchers at the University Medical Center Groningen, in collaboration with researchers from the University Medical Center Utrecht and University Medical Centers Amsterdam. The Knowledge Center for Palliative Care for Children, the Child and Hospital Foundation, all Children's Comfort Teams, all regional Networks for Integral Child Care and a parent panel were also involved.
