The Individual Care Plan in 2022

The currants in the porridge. Looking back on palpable enthusiasm and dedication.

In the spring, the test phase of the renewed Individual Care Plan (IZP) started as part of the IZP project . We look back on a year in which the enthusiasm and dedication of parents and healthcare professionals was strongly felt. Some of the people involved in the IZP project share their highlights with us!

Special project
During the test phase, we continuously worked hard to meet the needs of healthcare professionals and parents. To learn how to work with an ICP in an accessible way, training courses have been organized and a PowerPoint has been published in which the most frequently asked questions are answered. Healthcare professionals can get started quickly with an ICP.

“The IZP project is a special project for me. Just think of a ship that is large in size and complexity and yet progresses well! I think that's because people know how to find each other when necessary for progress and are flexible in doing so. By constantly making adjustments, we keep track of the end point! Those moments when we have to make adjustments are the icing on the cake for me.” - Eduard Verhagen, pediatrician and professor of pediatric palliative care

Foundation of proactive care
At the beginning of 2022, the original IZP was changed to IZP 2.0 based on input from parents and healthcare professionals. As a result, more attention has been paid to Advance Care Planning (ACP) in this renewed ICP. There is now room to describe the shared goals for care and treatment that have been discussed in ACP conversations with the child and parents. Treatment restrictions and wishes and agreements regarding the end of life can also be noted.

For me, a highlight of the ICP project is the increased awareness that ACP conversations are a foundation for proactive child and family centered care. I also see this reflected in the enthusiasm and eagerness to learn during the ACP training sessions.”
– Marijke Kars, senior researcher

Can not be missed
With no less than ten articles and interviews, the Individual Care Plan online was not to be missed! And that is also important if we want to achieve our goal: an ICP for every child with a life-threatening or life-limiting condition.

The IZP project has really come to life. This would not have been possible without the great efforts of all professionals and the increasing reputation of the IZP.”
– Chantal Joren, PhD candidate IZP project

Better care and greater awareness
The use of an ICP should improve care for the child and family. This is made possible in part by the good interaction between practice and scientific research and by the efforts of all those involved.

“The interaction between research and implementation took shape really well this year. This allows us to really improve the care for children and families.”
– Judith Aris, postdoc researcher child palliative care

Within the Networks for Integral Child Care (NIK), we as network coordinators see a greater awareness of the existence of an ICP. Professionals address each other if an ICP has not yet been drawn up and parents are proactively alerted to this. We also see enormous involvement from the professionals, despite the great pressure due to COVID-19 and staff shortages in child care.”
– Chantal van der Velden, network coordinator NIK Southwest and NIK Southeast

“It is great to be able to contribute as a project leader to optimizing integrated care through the further development of the IZP.”
– Ria Slingerland, project leader IZP project

Preview
There is also a lot to look forward to in 2023. Up to and including January, interviews and questionnaires will be used to investigate the experiences of parents and healthcare professionals with the IZP 2.0. Based on the results, IZP 2.0 will be further developed into the definitive IZP 3.0, which is expected to be used in the spring of 2023.

Does your child have an Individual Care Plan 2.0? Or do you work with it as a healthcare provider? Through www.izpproject.com it is still possible to register for the questionnaire and/or interview via the registration form for parents or care providers. You can also go directly to the questionnaire via the link below:
