Transition 18-/18+

Anchoring the professional group of Doctors with Intellectual Disabilities (AVG)

Around 5,000 children and young adults with intellectual disabilities require palliative care. The GDPR plays a crucial role in the transfer of these children to adult care. However, it is not self-evident that this expertise will become available to these children/young people and their families. As the specialist for this target group, the AVG is often not involved at all or only at a late stage. Within this project, we will work to strengthen the role of the GDPR and to permanently embed it in the new working method and transfer system of the Integrated Child Care Networks and Palliative Care Networks. The aim is to improve the assurance and continuity of care for the large group of palliative children with an intellectual disability during and after the 18-/18+ transition.
