Transition 18-/18+

Investigate bottlenecks in the funding and organization of adolescents and translation into the work agenda.

In order to gain more insight into bottlenecks, both in terms of funding and the organization of care and support, as soon as a young person turns 18, a study was started in June 2022 by order of the Knowledge Center for Pediatric Palliative Care and Palliative Care in the Netherlands (PZNL). In this way we also gain more insight from the parent and child perspective into what the care provision entails, the size of the potential care fall that this transition entails and which (system) bottlenecks young adults and parents encounter when it comes to organizing, having available and financing integrated care and support during and after the transition to the age of 18. Based on the research, a national Work Agenda Transition 18-/18+ will be drawn up:
