Digitization of the revised Individual Care Plan (IZP)

It is important that children, adolescents and their families experience support for advance care planning and the right care at the right time. The aim is for all families with a seriously ill child/young adult to have a digital ICP available from the diagnosis of a life-shortening or life-threatening illness. Care providers must be competent and knowledgeable to shape advance care planning and embed it in the ICP. It describes the wishes and needs of the child and family. These principles from the IZP are leading for the integral and transmural cooperation of all care providers in the 1st, 2nd and 3rd line. The IZP must always be available to the family and all care providers involved, with the child/young person and family in charge.

We want to achieve that an integral, cross-domain ICP is available for every child and family in palliative care. This must always be available and up-to-date for caregivers and family. In addition, the digitization of this plan is a requirement.
