Talking together about preferences and goals of care and treatment for the coming period is called Advance Care Planning (ACP). ACP conversations are conducted to get an optimal picture of the current situation and of the values, goals and preferences for the future. This is then translated into specific goals and agreements in the Individual Care Plan (IZP). ACP conversations therefore form the basis for completing an ICP. The ultimate goal is that the care and treatment is tailored to the perspective of the child and the family. In the context of the IMPACT project, various tools have also been developed to apply the ACP conversations in practice. These tools are available to the child, the parents and the healthcare professionals involved.

There was a need from the professional field for low-threshold information on this subject. That is why we have developed the animation video below that clearly explains Advance Care Planning and the associated tools.
