When a child needs palliative care, the family has a lot to deal with. The care for the child is intensive and is often provided by several care providers. Everything can be arranged.

The child and family, as well as all care providers involved, must be aware of the agreements and wishes that have been made and expressed. This may concern the care and treatment of the child, but also the quality of life and death. These wishes and agreements are laid down in the Individual Care Plan . In the animation below we explain in a simple way what an Individual Care Plan is, by whom it can be started and completed and when it is used.
